You should only need to provide one of the accepted documents. For an Illinois dental practice, the easiest options are generally an IRS EIN verification notice or an Illinois Certificate of Good Standing.
Option 1: Download an IRS EIN Verification Notice
You may be able to download an official CP575 EIN Verification Notice through your IRS Business Tax Account:

After signing in:
Select your business.
Open Tax Records.
Select the EIN Verification Notice.
Download and save the CP575.
The CP575 is an official IRS notice confirming your Employer Identification Number.
If you cannot access the notice online, call the IRS Business and Specialty Tax Line at 800-829-4933, Monday through Friday from 7:00 a.m. to 7:00 p.m. local time. Ask for Letter 147C, EIN Previously Assigned. You will need to verify that you are authorized to act on behalf of the business.

Option 2: Purchase an Illinois Certificate of Good Standing
You can purchase an official Certificate of Good Standing from the Illinois Secretary of State:

To obtain it:
Search for your practice using its exact legal business name.
Open the correct business’s File Detail Report.
Select the option to purchase a Certificate of Good Standing.
Pay the $25 fee.
Download the certificate from the receipt page or the email you receive.
Be sure to search for the legal entity name registered with the state, which may be different from the dental office’s public or marketing name.
Option 3: Request a Certified Copy of Your Formation Documents
You can also request a certified copy of your:
Articles of Incorporation, if your practice is a corporation or professional corporation
Articles of Organization, if your practice is an LLC or PLLC

Option 4: Purchase a Business Credit Report
You may also purchase a business credit report through one of the approved business credit bureaus.

Search using your practice’s exact legal name and Illinois address. This option will only work if the credit bureau already has a business credit profile for your practice.
The Form 8871, Form 8872, Form 990 and SEC filing options generally do not apply to a privately owned dental practice. Those documents are normally associated with political organizations, nonprofit organizations or publicly traded companies.
The fastest and least expensive option will likely be the $25 Illinois Certificate of Good Standing.
