Any time someone completes a form, the information is saved in your FormDr account for you to review, download, or export.

  1. Watch for the Notification
    When a patient or user submits a form, you’ll receive a notification letting you know a new submission is ready.
    This alert is just a heads-up. The form data itself stays secure inside your FormDr account.

  2. Log In to Your FormDr Account
    To view the actual submission, log in to your FormDr dashboard. All completed forms are stored securely there,
    so you can review them anytime in one central place.

  3. Open and Review the Submission
    Once logged in, select the form and open the submission you want to review.
    You can view all responses directly online in a secure environment.

  4. Export or Save Your Data (Optional)

From your dashboard, you can:

  • Save submissions as PDFs
  • Export data as CSV files
  • Sync submissions to Google Drive for easy storage and sharing
